Member Network Statement of Interest Form
This information is for interested persons in becoming a co-opted or appointed officer/committee member of a Member Network committee, sub-committee or working party. The following information is here to help you to submit your interest to the relevant committee and follow to Society procedure appropriately.
To put yourself forward you will need to complete both the statement of interest and declaration of interest forms.

Statement of interest
When a Member Network committee appoints co-opted members to serve on its committees, it has a recognised need to be well informed about the credentials for office of those who are to be appointed. It would greatly assist members of the relevant committee in their deliberations if you would please complete the statement of interest form.

Conditions of co-option or appointment
By completing the online statement of interest form you are agreeing that if co-opted or appointed to the post you agree to take on board the roles and responsibilities of the position.
By completing the online statement of interest form you are agreeing to comply by the policies and procedures of the Society.
Any illegible or omitted entry or signatures will invalidate the nomination.

The Board of Trustees has approved a Code of Conduct for members undertaking work for the Society. All Member Network committee and working party members are required to agree to the Code of Conduct.

Declaration of interest
This is a formal process that all Society committee and working party members need to complete, so that the Society can hold a register of members’ interests for all Boards, committees, standing committees and working groups in order to comply with Charity Commission guidelines. After you have completed your form and the deadline for registering interest has passed, your information will be sent to the relevant committee for them to consider the responses received.
Once the committee has made a decision has been made in relation to the roles, a member of the committee will contact you to inform you of their decision.
